Lasting Power of Attorney – Health & Welfare

This lets chosen people make decisions about your care and treatment. It allows your
attorneys to liaise with your GP, arrange long-term residential care, and make decisions
about medical intervention; although in the most grave circumstances, the medics may have
the final say.
Questions to think about:
Who understands your medical and personal wishes?
Do they live close enough to visit?
Should they have authority over life-sustaining treatment decisions?
It is important to think carefully about your wishes, and set them out in this document.
Doing so gives you peace of mind, and also eases the pressure on your attorneys when they
have to make life changing, or ending, decisions.
An LPA for Health & Welfare only takes effect if you lose capacity.
